- Right to a Paper Copy of This Notice: You have the right to a paper copy of this notice upon request, even if you agreed to receive this notice electronically.
Uses Requiring Your Authorization
We will obtain your written authorization before using or disclosing your PHI for purposes other than those described above (or as otherwise permitted or required by law). You may revoke any authorization at any time, in writing, except to the extent we have already taken action in reliance on your authorization.
Specifically, we will obtain your authorization before using or disclosing your PHI for marketing purposes, before selling your PHI, and before disclosing psychotherapy notes (if applicable).
Changes to This Notice
We reserve the right to change this notice and to make the revised notice effective for PHI we already have about you as well as any information we receive in the future. We will post a copy of the current notice on our website and at our place of business. The notice will contain the effective date on the first page.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the Department of Health and Human Services. To file a complaint with us, contact our Privacy Officer using the information below. You will not be penalized or retaliated against for filing a complaint.
